Government-issued vital records can include: birth certificates as well as marriage, divorce, or death records.

Identification
The Office of the State Registrar of Vital Statistics requires certain documentation as proof of identity. Please see the identification guidelines in English or Spanish.

Birth Records

Eagle County is able to issue certified copies of birth records for births occurring anywhere in Colorado from 1906 to the present. For births prior to 1906, certified copies can only be issued for births that occurred in Eagle County. All other certificates must be requested either from the county of birth or from the Colorado Department of Public Health. Please note that Colorado is a closed record state meaning that individuals who can provide proof of relationship or tangible interest in a birth certificate are eligible to purchase. Please see the eligibility requirements.

Birth certificates can be obtained by completing an application. The application can then be brought to our office in person or mailed. Please include a copy of your driver's license or passport, payment, and if applicable, proof of relationship. Credit cards are accepted with a 50 cent convenience fee for each application.

All mailed applications will be processed within 3 to 5 business days of receipt. Please mail applications to:

Applications may be hand delivered to our Eagle office for certificates to be mailed to the requester. Same day service is available Monday through Friday from 10 a.m. to 2 p.m. The cost of a birth certificate is $20 for the first copy and $13 each for additional copies.

Death Records

Eagle County can issue certified copies of death certificates for deaths occurring in Eagle County. Colorado is a closed record state meaning that individuals who can provide proof of relationship or tangible interest in a death certificate are eligible to purchase. Please see the eligibility requirements.
