The Healthy Families team oversees public health programming and services that support pregnant people and families with newborns. We provide resources and programming in a way that is compassionate and centers the experience and expertise of each family. All of our programming is voluntary, offered in English and Spanish, and at no cost to our community members.


Nurse Family Partnership
The Nurse Family Partnership is a nurse home visiting service for first time parents in Eagle County.  A nurse partners with families starting early in pregnancy and continuing until the child’s second birthday.

Family Connects Colorado

Family Connects is an in-home nurse service available to all Eagle County residents with a newborn. There are no eligibility requirements! This service is voluntary and offered at no cost to families. To learn more about the Family Connects program model and background, click here

Children with Special Health Care Needs

The Health Care Program for children and youth with special health care needs provides care coordination for children and youth from birth to age 21. Care coordination connects families with the information and resources they need to manage their child's health care. Any family can use care coordination services at no cost, regardless of income or diagnosis.

Women, Infants, and Children (WIC)
Women, Infants and Children (WIC) is the Special Supplemental Nutrition Program for Women, Infants and Children. WIC is the nation’s most successful public health nutrition program. We provide healthy food, nutrition education, and community support to income-eligible pregnant women, mothers of infants, and children up to 5 years old.
