Birth records
Eagle County Health & Human Services is able to issue certified copies of birth records for births occurring anywhere in Colorado from 1906 to the present. For births prior to 1906, HHS can issue certified copies for only those births that were in Eagle County.
All other certificates must be requested either from the county of birth or from the Colorado Department of Public Health.
Death records
Eagle County can issue certified copies of death certificates for deaths occurring in Eagle County.
Request for records
All requests for birth or death records must include a copy of the requestor’s identification. Please submit your written request with a copy of your driver’s license, state ID or passport. To learn what other documents may be acceptable for identification, call 970-328-8840.

Please note that additional copies must be ordered at the same time as the first copy.
