Eagle County Health & Human Services is able to issue certified copies of birth records for births occurring anywhere in Colorado from 1906 to the present. For births prior to 1906, HHS can issue certified copies for only those births that occurred in Eagle County. All other certificates must be requested either from the county of birth or from the Colorado Department of Public Health. Please note that Colorado is a closed record state meaning that individuals who can provide proof of relationship or tangible interest in a birth certifcate are eligible to purchase. Birth certificates can be obtained by completing an application
(second page is Spanish). The application can then be brought to our office in person or mailed. Please include a copy of your drivers license or passport, payment, and if applicable, proof of relationship (see eligibility list). Credit cards are accepted with a $.50 convienence fee for each application.

Applications may be hand delivered to our Eagle office for certificates to be mailed to the requestor. Same day service is available Monday through Friday from 10 a.m. to 2 p.m. The cost of a birth certificate is $17.75 for the first copy and $10 each for additional copies.
