CHIP:

The 2017 Community Health Improvement Plan (CHIP) was developed in partnership with community organizations and community members in an effort to address health concerns in Eagle County.  The effort identified five health improvement areas:  healthy living, independently aging in place, connection to preventative care, mental health and substance abuse, and radon awareness and mitigation. The 2017 CHIP provides an overview of Eagle County’s demographics and health indicators, which are tracked over time, and each of the five community priorities are provided with a road map on how to improve the top health issues over the next five years.

Health Equity Community Asset Map:

Promoting health happens long before you enter a doctor's office.  The social and environmental conditions in which people are born, grow, live, work, and age all impact an individual's opportunity for good health. Advancing health equity and eliminating health inequities is complex work, involving community members and public and private agencies. The Eagle County Health Equity Community Asset Map is a working document and compilation of existing community efforts aimed at addressing the social and environmental conditions which impact our most vulnerable populations. The map was created by Eagle County Public Health & Environment and many community partners.

Health Equity Graphic Novel: Start a Conversation about Health Equity

Created by the Center for Health Progress, Waiting for Health Equity is a graphic novel that aims to start new conversations about the complex challenges Colorado faces in working toward health equity. Available in both Spanish and English, the graphic novel is to be used as a tool to launch discussion and challenge perspectives and privilege around health equity. A facilitation guide is also provided for group conversation.
