The Well and Wise Program is a project of Eagle County Public Health and is a collaboration between Eagle County and the Consortium for Older Adult Wellness (COAW).
Through our partnership with COAW, many instructors have been trained to bring community-based chronic disease self-management (CDSMP) and falls prevention classes to adults 55 and older in Eagle County. Classes are also taking place in neighboring Summit, Garfield and Pitkin Counties. Check out their senior services programs for more information.
Well and Wise Classes include:
- Matter of Balance
- N’ Balance
- Tai Chi for Health
- Healthier Living Colorado – CDSMP
Another exciting component to this program is our partnership with local physicians to create a referral process for these classes, especially CDSMP to help patients become advocates of their health by working with their physicians on their journey to optimal health and self-management.
